What you will do:
– Conduct comprehensive assessments of individuals to determine their needs and devise tailored reablement plans
– Coordinate with multidisciplinary teams to deliver integrated and high-quality care services
– Monitor and evaluate progress, adapting support plans as needed to maximise independence
– Provide guidance and advocacy to service users, ensuring their voices are heard and their rights upheld
– Maintain accurate and timely records to comply with regulatory and organisational requirements
Requirements of the Social Worker:
– A degree or equivalent qualification in social work
– Current registration with Social Work England
– Significant frontline experience in UK Social Services as a qualified Social Worker
– Proven ability to work independently and as part of a team in a fast-paced environment
