We are on the hunt for a Band 6 Community Psychiatric Nurse to join our Older Adults Mental Health Team in Leytonstone, this is an ongoing locum role offering a generous rate of £27.23 per hour. You will work Monday to Friday, 9 am to 5 pm, immersing yourself in this vibrant, community-focused position.

What you will do:

• Manage a caseload for an identified group of patients, ensuring personalised and high-quality care.
• Conduct face-to-face assessments to understand patient needs and requirements.
• Take on duty work roles using your clinical expertise to make key decisions.
• Engage in CPA (Care Programme Approach), ensuring seamless and coordinated care.
• Utilise DIALOG for patient evaluations and feedback.
• Craft comprehensive care plans catering to individual patient needs.
• Review and adjust treatment plans to promote the best outcomes.
• Collaborate with various agencies to deliver integrated care solutions.
• Administer depot injections and support medication adherence.
• Mentor student nurses and junior colleagues, fostering a learning environment.
• Present patient cases at weekly MDT (Multi-Disciplinary Team) meetings, contributing to team discussions.
• Handle tribunal report writing with precision and accuracy.
• Facilitate safe hospital discharge, ensuring all precautions and follow-ups are in place.
