Located in the picturesque town of Pontyclun, we have an exciting opportunity for a Band 6 Occupational Therapist to join our Admission Avoidance Team. This ongoing full-time locum position offers a competitive hourly rate of £28. What you will do:

• Work alongside our Admission Avoidance, Stay Well @ Home Team in A&E departments and Acute Admission Units.
• Collaborate with a multidisciplinary team to assess patients and implement effective strategies to prevent hospital admissions.
• Develop and review care plans that promote patient independence and well-being.
• Provide expert advice and support to patients and their families.
• Manage a caseload efficiently, maintaining up-to-date records and ensuring high-quality care provision.
