Job Title: Band 6 Occupational Therapist – Care Coordination
Location: Camberwell, UK
Salary: Up to £28 per hour

What you will do:
- Take charge of and be accountable for a caseload as part of the community team, leading and managing the work of others when required.
- Coordinate comprehensive care plans by communicating effectively with other professionals involved in the care, ensuring regular reviews and updates are conducted as needed.
- Engage in collaborative communication with other teams, including in-patient services, to streamline and optimise care pathways.
To excel in this role, being a registered HCPC professional is essential, and you must also be a car driver to facilitate community visits and team coordination effectively.
