We are currently seeking a skilled and compassionate Band 6 Occupational Therapist to join the Discharge Planning Team in Worthing. Your Key Responsibilities:

  • Conduct comprehensive assessments of patients’ functional and environmental needs.

  • Formulate, implement, and review individualised discharge plans that ensure safe, timely, and effective transitions from hospital to home.

  • Collaborate with healthcare professionals including nurses, physiotherapists, and social workers to deliver integrated patient care.

  • Provide patient and family education regarding adaptive strategies, equipment, and post-discharge care.

  • Maintain accurate clinical records and contribute to discharge planning meetings and case discussions.

  • Conduct community visits where necessary to assess home environments and support discharge readiness.

Requirements:

  • HCPC registration as an Occupational Therapist

  • Valid UK driving licence – essential for home/community-based visits

  • Previous experience in acute discharge planning or hospital settings is highly desirable

  • Strong communication, time management, and decision-making skills
