What you will do:
– Work autonomously within the integrated multi-professional Intermediate Care team to deliver exceptional physiotherapy care.
– Manage your own caseload by assessing patients with complex and multiple pathologies using specialist clinical reasoning skills.
– Identify patient needs, establish goals, and provide tailored therapy interventions including adaptive equipment and rehabilitation programmes.
– Ensure patient and carer involvement in decision-making, keeping the patient at the heart of care.
– Provide expert support and education to peers, new staff, and students, enhancing team knowledge.
– Supervise junior staff and students, overseeing patient interventions and ensuring Assistant Practitioners and Integrated Care Workers maintain high standards of care.
– Engage in teaching and support to help Assistant Practitioners and Integrated Care Workers achieve their competencies.
– Take responsibility for a delegated caseload managed by Integrated Care Workers and Assistant Practitioners.
– Prioritise all referrals based on clinical needs and allocate or guide them appropriately.
– Provide excellent standard of care while working in various community settings, with access to Advanced Practitioners when needed.
Applicants must be registered with the Health and Care Professions Council (HCPC). A minimum of two years’ post-qualification experience in a relevant clinical setting is required. A full UK driving licence and access to a vehicle for work purposes are highly desirable.
