What you will do:
- Provide expert management of acute respiratory and medical conditions, ensuring comprehensive assessment and tailored interventions for each patient.
- Facilitate effective and safe discharges, planning with a multidisciplinary approach to ensure seamless transitions from hospital to home care.
- Collaborate closely with a team of healthcare professionals to optimise patient outcomes and promote respiratory health.
- Evaluate patient progress and adapt care plans as necessary to achieve the best possible results.
- Educate patients and their families on respiratory health and wellness to encourage proactive management.
Applicants should be registered with the HCPC and possess a full, valid UK driving licence.
