The salary for this permanent Clinical Pharmacist job is £65,805 – £73,304 per annum.

Main Duties:

  • Assist the lead pharmacists in delivering a high-quality, patient-centered medicines optimisation service according to national standards within a specified area.
  • Oversee and assess patients’ therapeutic programmes, offering expert information, advice, and guidance on the safe, effective, and cost-efficient use of medicines to ensure optimal pharmaceutical treatment with minimal adverse effects.
  • Perform medication reviews, including reconciliation for newly admitted patients and care planning before discharge, to minimise risks associated with medication transfers between care settings. Collaborate with medical and nursing staff to resolve clinical issues and ensure appropriate medication for each patient.
  • Work as part of multi-professional health and care teams, including attending ward rounds, to provide highly complex clinical pharmacy advice and expertise to peers, patients, and management, promoting and delivering best practices to patients.
  • Offer guidance on complex prescribing issues to medical and nursing staff to ensure compliance with legislative requirements and best practice standards in pharmacy service delivery.
  • Assist the Clinical Pharmacy Manager in developing local clinical pharmacy guidelines and policies. Implement national and local policies and guidelines related to medicines.
  • Provide specialist information to support formulary decisions by the Drugs and Therapeutics Committee.
  • Provide planned sessional clinical and professional supervision within the pharmacy dispensary.
  • Develop and deliver medicines-related training programmes to medical, nursing, and pharmacy staff.
  • Identify and participate in research areas, including audits of medicines use, to assist with medicines optimisation throughout the department.

Requirements of the Clinical Pharmacist:

  • Master’s degree in Pharmacy (four-year program).
  • Registration with the General Pharmaceutical Council (achieved by completing a one-year postgraduate pre-registration training and passing the final examination).
  • Postgraduate certificate in Clinical Pharmacy (completion of the first year of a postgraduate Diploma/MSc) or completion of the RPS postgraduate Foundation Programme.
  • Specialist knowledge of therapeutics for common disease states and frequently used therapeutic regimes.
  • A minimum of 2 years post registration experience in hospital practice, which should include significant experience in clinical pharmacy.
  • Experience of working effectively within a multidisciplinary team. Facilitation and tutoring of pharmacy staff.
  • Well-developed understanding of pharmaceutical technical and clinical procedures, acquired through training and experience.
  • Knowledge and experience in checking dispensed prescriptions and supervising pharmacy staff.
  • Strong understanding of medicines governance, safety, and management, including relevant legislation.
  • Familiarity with clinical governance, medication incidents, and risk management.
  • Up-to-date knowledge of national guidelines and legislation related to medicines use and pharmacy practice.
  • Practical knowledge of maintaining records in accordance with confidentiality and data protection protocols.
