Job Purpose:
Sanctuary Personnel are seeking a Community Nurse Practitioner to join our client’s Adult Mental Health Team. In this role, you will serve as a Care Coordinator, providing specialised, evidence-based assessment and treatment to clients with complex mental health needs. Your responsibilities will include:
- Acting as a lead in risk management for mental health presentations.
- Utilising advanced skills within a specialised nursing area to support our team.
- Delivering psychosocial education to individuals and families.
- Implementing safe, lawful interventions based on positive risk management assessments and collaborative care.

Person Specification:
- Registered Nurse (RMN) with at least 4 years post-qualification experience, including 3 years in a specialty area.
- Mentorship or Teaching Award/Qualification.
- Relevant post-qualifying specialty award.
- Proficient in Information Technology.
- Possess a full driving licence.
- Adherence to the NMC revalidation process.
- Enhanced skills in mental health provision, with a collaborative approach in engagement, assessment, and planning.
- Competence in medico-legal contexts of mental health care, including relevant laws and regulations.
- Ability to communicate effectively with diverse stakeholders and agencies.
- Experience in clinical event reviews and reflective practice.
- Competency in clinical decision-making and escalation processes.
- Ability to evaluate clinical outcomes and support informed decision-making.
- Demonstrated expertise in managing complex mental health issues and coordinating care across services.
