What you will do:
- Provide specialist assessment and equipment provision to promote single-handed care.
- Collaborate closely with health and social care teams to foster effective communication and facilitate patient discharges.
- Conduct proactive reviews aimed at enhancing care support through innovative equipment and advice.
- Serve as an essential link to prevent unnecessary hospital admissions, focusing on prevention and early intervention.
- Empower care staff by offering guidance and support, ensuring optimum care package delivery.
Requirements:
Qualified candidates must be registered with HCPC and have a minimum of 2 years’ experience in a related specialism. A driving licence is preferable, given the nature of the community-based role.
