Main Responsibilities

Case Management: Manage a varied caseload of children and young people with disabilities, ensuring they receive appropriate and timely support.

Assessment & Care Planning: Undertake thorough assessments and develop tailored care plans to meet individual needs.

Direct Work: Engage directly with children and families, ensuring their views and wishes are central to all decisions.

Safeguarding: Promote and safeguard the welfare of vulnerable children, adhering to statutory requirements and best practice.

Interdisciplinary Collaboration: Liaise effectively with professionals across health, education, and voluntary sectors to deliver holistic support.
Candidate Requirements

Qualifications: A recognised degree in Social Work and current registration with Social Work England.

Experience: Ideally experienced in working with children with disabilities, though strong experience in other statutory children’s services will also be considered.
